I think part of it is that the cape portion actually splits, so it has two wings -- or arms, or whatever you would like to call it -- as opposed to just one train. So that makes it look like her hips have a very fancy mustache. The dress itself could've been cool unmolested by trousers. I feel like over the years she's gotten crafty with certain tweaks but sticks with the same basic silhouette. It works, but it's probably not going to stay with me tomorrow. Lipstick, though! I would argue it's bad in both contexts. The peachy color of the illusion netting is... alarmingly corpse-like to me. Beyond which, I'm glad she was thrilled to put this on and felt lovely in it, but I think there are a hundred other dresses that would still hug her wonderful curves without also being hideous. It's uncanny. And unfortunate. She's young yet, though, and she's gone through a lot with her body and her family...
