I assume all those men were stuck dumb, like, "Oh no, our protocol briefing doesn't cover what to do here. Do we....go wrench her shoe out for her? NO THAT CAN'T BE RIGHT." The good news is, the Duchess is capable of yanking her heel out of a grate like every other human being and saved herself.
